Botox Membership Visit
Description
Subscription Details: I understand that the ID WELLNESS Botox Subscription costs $100 per month. This subscription allows me unlimited Botox treatments to the upper third of my face. Non-Transferable Subscription: I acknowledge that this subscription is non-transferable and is solely for my use. Additional Treatments: Should I require Botox treatments in other areas, I will pay per unit, based on the office's current price at the time of application. Automatic Renewal: I understand that this subscription will automatically renew each year on the date of initial consent. To cancel, I agree to notify ID WELLNESS at least 30 days before the renewal date. Missed Payment Penalty: If my account lacks sufficient funds at the time of billing, I am aware that a $35 fee will be charged for missed payments. Consent at Each Appointment: I agree to sign a consent form at every appointment, documenting the exact amount of Botox units used. Early Cancellation Policy: In case of early cancellation, I agree to pay for the value of the units received during the year up to the cancellation date. The cost per unit will be calculated based on the price at the time of cancellation.
